In photography, the effect is created by putting the camera on the macro setting, with light(s) on the background, and focuss the camera on another object in front of the lights.
You can also create the effect in photoshop afterwards, by using the rounded brush to create the bokehs, if you want them blurred, you can use a blur function in the filter section.
